Treatment Protocol in Our Clinic
1. Blood Sugar Investigation: This is a must investigation to find generalised sugar control of the patient. Can be done with portable glucometer also.
2. Meeting Our Doctor In Charge: Our doctor will talk to the patient about his problems, notes down the history and findings. He also examines the patients for sensory examination, joint mobility and gait analysis and type of ulcer - site, depth, presence of necrosis or infection etc.
3. Sensory Assessment The Foot: It is done for both feet for various sensations including soft touch, vibration and temperature. Once found many a time the patients and their relatives get a surprise by these findings. Many patients after knowing these results become more cooperative for treatment and take care of their foot care.
1. Joint mobility is checked to find out the rigidity of the joints and the cause of the stiffness and cause of the ulcer. Depending on the findings the operative plans will be decided.
2. Gait analysis is the most important of the examination. It will suggest which muscles are weak and which are tight. Depending on that surgical release of the tendons or tightening is planned.
3. Ulcer Examination: the doctor in charge will examine the ulcer for its size, site, depth and presence of infection or dead tissue in it. Depending on the findings we will decide about the future type of surgery - debridement, reconstructive or amputation. We also decide if the patient will be given outpatient treatment or indoor admission is required
4. Pedobarogram: it is a special study on the digital platform on standing and walking phase of the patient. It digitally measures the pressure over the whole sole. Gives reports in the form of numbers, colours and barogram. This is the one investigation based on which the footwear or the type of surgery is decided.
5. Vascular Doppler Study: If the pulse of the foot is normal usually arterial doppler is not advised. But if clinical doubt of decreased circulation is found, the arterial colour doppler is done. This will give a rough idea of the blood flow in foot and leg.
6. CT Angiography: if the arterial doppler is abnormal CT scan angiography is done to find out the disease level, distal runoff and the type of blockage. Depending on these findings we decide if the patient will need angioplasty or vascular bypass or amputation is more helpful.
7. Final Treatment
- Blood Sugar Control: if your sugar control is not good the case is then referred to his physician or endocrinologist for sugar control as this is one major factor which decides the rate of infection and healing.
- Depending on the findings in ulcer we will advise OPD treatment or indoor admission is advised. Depending on the finding either debridement, scrapping or reconstruction is planned for you.
- If you are ready for OPD intervention like flexor tenotomy, ingrown toenail excision etc. we will perform the procedure then only
- If anaesthesia is required, Your blood reports and other fitness for surgery is taken and surgery is planned and the date is given. you need to get admitted in hospital and Dr Aashutosh Shah will operate upon you for the concerned problem.
- If your case needs emergency services we will advise you immediate admission, control your blood parameters and take you for either debridement, amputation or incision and drainage.
8. After healing you will be provided with either therapeutic or preventive footwear.
9. Our clinic supplies all types of footwears, cushions, silicon soles for your comfort. If the patient with an amputation needs to improve his look, the cosmetic prosthesis is also available. The prosthesis for walking is also supplied to patients with above and below-knee amputations.
